I, authorize Family Worship Center to share with the Pastors  the information disclosed in the testing procedures, questionnaires, and interview process.

I understand that the purpose of discussion among the Pastors is solely to evaluate my suitability as a potential Lay Minister of Family Worship Center and that my information will be treated with sensitivity and confidentiality. 


I, authorize Family Worship Center personnel to conduct background checks on me, if one does not already exist or is in need of updating, in the church database. Additionally, I give permission for the resulting information to be disclosed to the Pastors of Family Worship Center. 

Confidentiality
A note about confidentiality and the nature of some of these questions:
The biblical qualifications have much to say about the character of a man’s marriage and family. These questions are designed to give us a broad picture as we consider you as a Lay Minister. We are aware that some of the following questions are personal and sensitive. We do not expect any person’s past to be perfect or “clean.”  Our concern is focused on unresolved issues from the past or current matters. These issues are sensitive because, unless properly dealt with, they have the power to harm you as well as those to whom you would minister.  We ask these questions both for your protection and ours as a church, but especially for those among you to whom you intend to minister. Your answers on this form will only be read by the Pastors of Family Worship Center. We appreciate your honesty as you and your spouse answer these questions
